Completing a CFA-2 Form

 

1. If the Administrator Home page isn't already displayed, click the my admin homepage link near the top of the current page.

 

The Administrator Home page is displayed.

 

2. In the Quick Links section, click the add cfa-2 link.

 

The Statement of Organization for PAC or Legislative Caucus (CFA-2) page is displayed.

 

Note: Required fields are indicated by an asterisk * to the right of the label and must be completed.

 

1. In Section A of the form, complete the Committee information and click the next button. To close the page without submitting the form, click the return button. To indicate the Committee is a PAC, select the no check box in box 11.
   
   To indicate the Committee is a Legislative Caucus Committee, click the yes check box in box 11.

 

The remainder of Section A and Section B of the form are displayed.

 

2. Complete the rest of the Committee information and the Appointment of Treasurer information and click the next button. To return to the previous page, click the previous button.
   
   To close the page without submitting the form, click the return button.

 

Sections C and D of the form are displayed.

 

3. Complete the Acceptance of Appointment information and the Certification of Statement information.

 

4. Click the save button. To return to the previous page, click the previous button.
   
   To close the page without submitting the form, click the return button.

 

The information is saved and the Confirm Statement of Organization page is displayed. The system assigns a confirmation number to the transaction. Record this number so you can check the status of your Statement of Organization form.

 

5. Follow the instructions on the screen to complete the online filing process.

 

6. After you complete the online filing process, click the finish button.
