You can access the File a Gifts and Honoraria Report page to file a gifts and honoraria report. The report is used to disclose gifts, honoraria, and other benefits donations in excess of fifty three dollars that were given as a gift.
To access the Gifts and Honoraria Report page:
If the Administrator Home page isn't already displayed, click the my admin homepage link near the top of the current page.
The Administrator Home page is displayed.
In the Quick Links section, click the add link to the left of Gifts and Honoraria Report for Non-Registered Users.
The File a Gifts and Honoraria Report page is displayed.
In the Filing Period section perform the following:
Select the reporting year from the year drop-down list box.
Select the quarter the gifts, honoraria, or other benefits were received from the time period radio buttons.
Perform the following in the Officeholder Information section:
Type the Officeholder's first, middle, last name, and suffix in the corresponding fields.
Type the Officeholder's address in the address fields.
Type the Officeholder's city/state/zip in the city/state/zip fields.
Type the Officeholder's email in the email address field.
Type the Officeholder's phone number in the phone field.
Select the Officeholder's jurisdiction from the jurisdiction drop-down list box.
Select the Officeholder's office from the office drop-down list box.
Select the Officeholder's district from the district drop-down list box.
In the Gift, Honoraria, and Other Benefits section, perform the following for each gift, honoraria, and other benefits you want included in the report:

Click the I have nothing to report radio button. |
You have a gift, honoraria, or other benefit to report... |
Click the I have received the following gifts, honoraria, or other benefits during this period radio button. |
Type the name of the person who gave a gift, honoraria, or other benefit n the name of person giving field.
Type the date the gift, honoraria, or other benefit was received in the date received field.
Type a description of the gift, honoraria, or other benefit received in the description field.
Click the add item to list button.
To file the report, click the submit
button. To
close the page without filing the report, click the cancel
button.
