You can access the Notice of Candidate/Committee page to add or amend a Candidate affidavit or register a Committee.
To access the Notice of Candidate/Committee page:

Perform the steps for Searching for a Candidate Affidavit.
On the Notice of Candidate/Committee page, perform the following if you are adding an affidavit for a new Candidate:
Type the Candidate's first name in the first name field.
Type the Candidate's middle name in the middle name field.
Type the Candidate's last name in the last name field.
Select the Candidate's name suffix from the suffix drop-down list box (e.g., Jr., Sr.).
Type the first line of the Candidate's physical address in the address field.
Type the second line of the Candidate's physical address in the address2 field.
Type the Candidate's physical city/town, state, and ZIP code in the city/town, state and zip code field.
Type the Candidate's primary phone number with area code in the phone number field.
Type the Candidate's fax number with area code in the fax number field.

If the Candidate filed an Accepted Voluntary Spending Limits form, select the accepted voluntary spending limits check box. If the candidate did not file an Accepted Voluntary Spending Limits form, clear the accepted voluntary spending limits check box.
Select the party that supports the Candidate from the party drop-down list box.
Select the election in which the Candidate will be running from the election cycle drop-down list box.
If you are adding an affidavit for a new Candidate, perform the following:
Select the office for which the Candidate will be running from the office drop-down list box.
If the office is for a specific district, select the district from the district drop-down list box.
Type the Candidate's email address in the email address field (e.g., jdoe@myprovider.com).
Type an alternate email address for the Candidate in the alternate email 1 field.
Type another alternate email address for the Candidate in the alternate email 2 field.
To generate a scannable label, select the generate scan label check box. If you don't want to generate a scannable label, clear the generate scan label check box.
Click the save button to save the information. To close the page without saving your changes, click the cancel button.

Click the submit button to save your changes. To close the page without saving your changes, click the cancel button. If the affidavit is for a Candidate that has multiple affidavits, the current candidate record is not updated unless the affidavit is the most current one.

In the Select Committee Type section, perform the following:
Select the radio button that corresponds to the type of Committee you want to register. The available choices are:
Candidate Committee – Person, including a candidate, or persons with the common purpose of receiving contributions or making expenditures under the authority of a Candidate.
Political Committee – Any person, other than a natural person, or group of two or more persons, including two or more natural persons that have accepted or made contributions or expenditures in excess of $200 to support or oppose the nomination or election of one or more Candidates.
Federal PAC – A political Committee that has registered with the Federal Election Commission (FEC).
Political Party Committee – Any group of registered electors who, by petition or assembly, nominate Candidates for the official general election ballot.
527 Political Organization – Political organization defined in section 527(e)(1) of the Federal Internal Revenue Code that is engaged in influencing or attempting to influence the selection, nomination, election or appointment of any individual to any state or local public office in the state and that is exempt, or intends to seek an exemption, from taxation pursuant to section 527 of the internal revenue code.
Issue Committee – Any person, other than a natural person, or any group of two or more persons, including natural persons, that has a major purpose of supporting or opposing any ballot issue or question and has accepted or made contributions or expenditures in excess of $200 to support or oppose any ballot issue or question.
Small Donor Committee – Any political Committee that accepts contributions from only natural persons who each contribute no more than $50 in aggregate per year.
Independent Expenditure Committee – A political Committee that only makes independent expenditures, without coordinating with campaigns, political parties, or other outside groups.
In the Registration section, perform the following:
Type the Committee's name in the committee name field.
Type the first line of the Committee's physical address in the address field.
Type the second line of the Committee's physical address in the address2 field.
Type the Committee's physical city/town, state, and ZIP code in the city/town, state and zip code field.
Type the Committee's primary phone number with area code in the phone number field.
Type the Committee's fax number with area code in the fax number field.

Type the purpose of the Committee and the
Candidate supported in the purpose
field. Be
specific in describing the Committee’s purpose.
A Candidate Committee shall identify the name of the Candidate
and the specific elective office sought. A political Committee
or small donor Committee must identify the types of Candidates
being supported or opposed, such as party affiliation or public
policy position and if known, specific Candidates being supported
or opposed. Issue Committees should be a specific as possible
in identifying the ballot issues or questions they choose
to support or oppose and may not use generic phraseology once
an issue they are supporting or opposing is known.
For a Candidate Committee, perform the following:
If the office sought is a partisan position, select the party the Candidate is affiliated with from the party drop-down list box.
Select the election in which the Candidate will be running from the election year drop-down list box.
Select the office for which the Candidate will be running from the office drop-down list box.
If the office is for a specific district, select the district from the district drop-down list box.
Type the name of the Committee's financial institution in the institution name field.
Type the first line of the financial institution's address in the address field.
Type the second line of the financial institution's address in the address 2 field.
Type the financial institution's city/town, state, and ZIP code in the city/town, state and zip code field.
In the Registered Agent section, perform the following:
Type the Registered Agent's first name in the first name field.
Type the Registered Agent's middle name in the middle name field.
Type the Registered Agent's last name in the last name field.
Select the Registered Agent's name suffix from the suffix drop-down list box.
Type the Registered Agent's phone number with area code in the phone number field.
Type the Registered Agent's alternate phone number with area code in the alternate phone number field.
Type the Registered Agent's email address in the email address field (e.g., jdoe@myprovider.com).
Type an alternate email address for the Registered Agent in the alternate email 1 field.
Type another alternate email address for the Registered Agent in the alternate email 2 field.
In the Designated Filing Agent section, perform the following:
Enter the Designated Filing Agent's name in the corresponding fields.
Type the Designated Filing Agent's phone number with area code in the phone number field.
Type the Designated Filing Agent's email address in the designated filing agent email field (e.g., jdoe@myprovider.com).
If desired, enter alternate email addresses for the Designated Filing Agent in the alternate email 1 and alternate email 2 fields.
For an Independent Expenditure Committee, perform the following for each parent corporation, subsidiary corporation, or affiliated labor organization on whose behalf you are registering the Committee:
In the Parent, Subsidiary, DBA Names, and Other Associated Organizations section, select the radio button that corresponds to the type of parent corporation, subsidiary corporation, or affiliated labor organization.
Type the name of the parent corporation, subsidiary corporation, or affiliated labor organization in the name field.
Click the add to list button.
The parent corporation, subsidiary corporation, or affiliated labor organization is added to the data grid. To remove a parent corporation, subsidiary corporation, or affiliated labor organization from the registration; click the corresponding remove link.
Click the save button to save the information. To close the page without saving your changes, click the cancel button.
The Committee Registration page is displayed. The Colorado Secretary of State’s office will review your registration and if approved, you will receive log on information by the next business day.
